I do know most of the theories but is it important to actually refer them in the exam? Ie do I have to learn all of them by heart?

You get more marks by structuring your answers round a suitable theory. You will not normally be asked to describe a theory (and if you find you are, you are probably answering the question incorrectly), but you are expected to apply theories as appropriate.

Is it necessary to state who came up with the theory, or is it sufficient to know the theory alone?

If you know the originator of the theory or its name then you have a shortcut. What questions don’t want you to do is describe a theory: they want you to use it, and if you can’t remember what the theory is called then you are in danger of having to describe it to let the examiner know what theory you are using.
