signature of my mentor is a requirement or not.

Hi, can anyone tell me if before the uploading of the rap, we must have our mentor's signature on a particular form to be sent to OBU? And should we inform the OBU beforehand that I'll submit my project in period 29 or 30.If yes, what r the procedures? thanks
