Sequence of the report & cover page

Hi there! Can anyone please advice me on the sequence of the report? I have no idea where to include SLS.
Also, is there a specific format for cover page? I looked it up online but it seems that everyone has different way of doing it.

The report file contains just that -report and nothing else and no, there is no prescribed format for the cover but Topic title, company, ACCA Reg. No and word count are the norm.
You load all your files including the SLS and references as separate MS Word files
