Is it possible that right issue increases the number of shareholders in an entity or it always offers to existing shareholders?
A rights issue, by definition, is only offered to existing shareholders.
(Although, as you will discover in Paper F9. if any shareholders to not want to take up their rights they can sell them to other people and they can take up the shares. So there can end up being more shareholders.)
