OBU Resubmission Form

I failed my RAP but passed the SLS. Now I need to resubmit my RAP but have changed the topic while the organisation still remains the same. The resubmission form says “You MUST include this form with your resubmitted project unless you have changed your topic/organisation”. Does it mean I don’t have to submit the resubmission form and where do I specify that I have changed the Topic?
