Hello! Could anyone please explain me how to embedding risk in the system, I understand how to embed risk in the culture bu having a proper reporting system, not having a culture of blame bad news to be reported as soon as possible but im not clear what embedding risk in the system and processes means . Pls give me an example to make it easier to understand
