Appraisal of individual directors, Audit Committees and Board as a whole

As we know that the performance of Executive Directors is measured with the regular meeting of NED and Chairman. Similarly, performance of Chairman is appraised with regular meeting of NED and Executive Directors without the Chairman. But, how is the performance of individual director and the board of directors as a whole is measured?
