I had similar issue , i passed my P4 paper ( last ) with score of 50 but my name was not included in Affiliate list , however i never had any issue with regards to login to MyAcca and i had Affiliate theme to MyAcca with ” Afilliate ” written next to my ACCA user number.

I chat with Acca online chat yesterday and today my name was there in affiliate list.

If you can access MyACCA , there will be an option “Administer your account and pay your fees” , just click that and new window will popup with an option to include your name is Affiliate list , just check that checkbox and save it , then allow the change to take effect for a day and you will have your name in the list.
